The purpose of this study was to map out the knowledge, competence and documentation-practises amongst nurses regarding the treatment of chronic wounds among patients in primary healthcare. Data was collected through a knowledge questionnaire survey and through an examination of journal data belonging to 12 patients receiving wound treatment and was carried out in spring 2019 in one Norwegian municipal. The study includes 47 responses (response rate 35%) and 322 wound journal reports. Data was analysed by descriptive statistics. Only 4 % of the nurses had sufficient knowledge level regarding wound assessment. 67% of the reports (N=322) had one or none TIME/S categories rated. There was a significant relationship between TIME/S categories rated and documented “completion of wound treatments” (P = 0,016) only among those who had documented 0 or 1 wound category. The results suggest that nurses didn’t always assess and document the wounds according to the TIME/S-model categories.
