Abstract
Årsak/hensikt: Nedre luftveisinfeksjon er en ledende årsak til død hos eldre pasienter på sykehjem. Nedre luftveisinfeksjon er også kilde til et stort forbruk av antibiotika i denne pasientgruppen. Vi ville finne ut om nasjonale retningslinjer for antibiotikabruk i sykehjem ble fulgt, og også hvordan forløpet av en nedre luftveisinfeksjon på sykehjem ser ut.
Metode: Vi brukte en cluster- randomisert studie som fulgte et stepped wedge- design. Vi inkluderte 28 sykehjem med til sammen 272 pasienter. Alle disse pasientene ble behandlet på sykehjemmet med per orale antibiotika. Pasienter som ble lagt inn på sykehus i forløpet, eller som er behandlet med intravenøs antibiotika, er ekskludert her. Oppfølgingstiden etter symptomdebut var 14 dager. Vi benyttet oss av flere ulike skjema for å dokumentere forløpet og utfallet.
Resultat/konklusjon: Nasjonale retningslinjer ble fulgt i forskrivning av antibiotika hos de fleste sykehjemmene. Vi fant ingen klinisk viktige forskjeller mellom kvinner og menn i analysene. Risiko for utvikling av delir av en nedre luftveisinfeksjon var 7 %. 62 % av pasientene var tilbake i normaltilstand etter endt forløp, og mortalitetsraten var 9 %.
Purpose: Lower respiratory tract infections are a leading cause of death in elderly patients at nursing- homes, and also a source to extensive use of antibiotics. We wanted to study if the Norwegian national guidelines for prescribing antibiotics in nursing- homes were followed, and also study the outcome of patients in nursing homes with lower respiratory tract infections.
Methods: We used a cluster- randomized study which followed a stepped wedge- design. 28 nursing- homes, and a total of 272 patients with lower respiratory tract infection, in the county of Vestfold in the south of Norway were included. We excluded those patients who got hospitalized during their time of infection, and all the patients who did not get oral antibiotics. The follow-up period was 14 days after the debut of symptoms. We used different forms to get information about the course and outcome in our patients.
Results/conclutions: National guidelines were followed in the prescribing of antibiotics in most of the nursing- homes. We did not find any clinically important difference between the sexes in any of our analysis. We found that the risk for delirium was 7 % for lower respiratory tract infections. 62 % of our patients were back to their normal condition after 14 days, and the mortality rate was 9 %.
