Sammendrag
Bakgrunn: Det er økende bevis for at lipid-innholdet i hjertet (IntraCardiomyoCellular Lipid, ICCL) er involvert i etiologien av dilatert kardiomyopati og hjertesvikt. Målet med denne studien var å undersøke effekten av forhøyede plasma nivåer av frie fettsyrer (FFA) på lipid-innholdet i hjertet og på hjertets funksjon. I tillegg ble også effekten av akutt trening på ICCL-innholdet undersøkt. Utvalg og Metoder: Ni friske slanke unge menn (BMI: 23.2 ± 2.8kg/m2; alder: 24.9 ± 3.7år) ble rekruttert i en randomisert kryss-over studie. ICCL-innholdet ble bestemt før trening, umiddelbart etter trening, og igjen 4 timer etter restitusjon fra trening, av magnetisk resonanse spektroskopi (MRS), i en tilstand med høye plasma nivåer av FFA (sykling og restitusjon i fastende tilstand) og i en tilstand med lave nivåer av FFA (sykling og restitusjon med glukose supplement). Hjertets funksjon og energistatus ble bestemt på slutten av hver testdag (post-restitusjon). Resultater: ICCL innholdet økte signifikant med 77% ved forhøyede nivåer av FFA i plasma (fra 0.22 ± 0.03% til 0.39 ± 0.05%, i baseline vs. post-restitusjon målinger, henholdsvis, P = 0.011), mens derimot denne økningen i ICCL ikke var observerbar i tilstanden med lave nivåer av FFA (fra 0.27 ± 0.03% til 0.22 ± 0.06%, P = 0.309). Videre, ICCL innholdet var høyere etter restitusjon i fastende tilstand sammenliknet med glucosetilstanden (0.39 ± 0.05% og 0.22 ± 0.06%, henholdsvis, P = 0.040). ICCL-innholdet viste ingen significant forandring med trening i noen av de to tilstandene (fra 0.22 ± 0.03% før og 0.30 ± 0.07% etter trening for fastende tilstand, P = 0.181, og 0.27 ± 0.03% før og 0.24 ± 0.06% etter for glucosetilstanden, P = 0.531). Ejections fraksjonen viste en samlet lav systolic hjertefunksjon, og som videre var significant lavere i glucose tilstanden sammenliknet med den fastende tilstanden (48.98 ± 0.92% og 55.84 ± 1.68%, for glukose- og fastende tilstand henholdsvis, P = 0.002). Konklusjon: ICCL-innholdet i hjertet hos friske slanke unge menn var betydelig økt etter forhøyede plasma nivåer av FFA, mens denne økningen var forhindret i glukosesupplementert tilstand. Dette tyder på at plasma konsentrasjonen av FFA er en viktig determinant for ICCL, som igjen tyder på at opptaket av FFA i hjertet ikke er strengt regulert, men i stor grad avhenger av tilgjengeligheten av FFA. Videre er det ingen forbindelse mellom systolic hjertefunksjon og høyt ICCL-innhold sett med denne protokollen, noe som tyder på at ICCL-innholdet ikke er den viktigste determinanten for systolic hjertefunksjon. Men flere parametre for hjertets funksjon bør undersøkes før en klar konklusjon kan trekkes.
Background: There is increasing evidence that Cardiac lipid content (IntraCardiomyoCellular Lipid, ICCL) is involved in the aetiology of dilated cardiomyopathy and heart failure. The goal of the present study was to examine the effect of elevated plasma free fatty acid (FFA) levels on cardiac lipid content and cardiac function. Additional, the effect of acute exercise on ICCL content was also investigated. Selection and methods: Nine healthy lean young men (BMI: 23.2 ± 2.8kg/m2; age: 24.9 ± 3.7yr) were recruited in a randomized cross-over trial. ICCL content was determined before exercise, directly after exercise and again after a 4-hour recovery from exercise, by magnetic resonance spectroscopy (MRS), in a condition with high plasma FFA levels (cycling and recovery in fasted state) and in a low FFA condition (cycling and recovery with glucose supplementation). Cardiac function and energy status was determined at the end of each the test day (post-recovery).
Results: ICCL content increased significantly by 77% with elevated plasma FFA levels (from 0.22 ± 0.03% to 0.39 ± 0.05%, in baseline vs. post-recovery measurements, respectively, P = 0.011), whereas this increase in ICCL was not observable in the condition with low levels of FFAs (from 0.27 ± 0.03% to 0.22 ± 0.06%, P = 0.309). Further, ICCL content was higher post-recovery in the fasted condition compared to the glucose condition (0.39 ± 0.05% and 0.22 ± 0.06%, respectively, P = 0.040). ICCL content did not change significantly with exercise in either of the two conditions (from 0.22 ± 0.03% before and 0.30 ± 0.07% after exercise for the fasted condition, P = 0.181, and 0.27 ± 0.03% before and 0.24 ± 0.06% after for the glucose condition, P = 0.531). The ejection fraction showed an overall low systolic cardiac function, which was also lower in the glucose state compared to the glucose state (48.98 ± 0.92% and 55.84 ± 1.68%, for glucose- and the fasted condition respectively, P = 0.002).
Conclusion: The accumulation of triglyceride in the myocardium of healthy lean young subjects was significantly increased after plasma elevated FFA levels, whereas this increase was prevented in the glucose fed state. This suggests that plasma FFA concentrations is an important determinant of ICCL, which in turn suggests that uptake of FFAs in the heart is not strictly regulated, but largely depends on the fatty acid availability. Furthermore, there is no association between systolic cardiac function and high ICCL content seen with this protocol, which suggests that cardiac lipid content seems not to be the main determinant of systolic cardiac function. However more parameters of cardiac function should be studied before a definite conclusion can be drawn.
