Sammendrag
Bakgrunn og formål:
Gange på skråstilt fortau og terreng med det høyeste nivået under protesefoten er rapportert som spesielt anstrengende for transfemoralt amputerte. I denne studien undersøkes forskjell i energiforbruk og opplevd anstrengelse ved gange på tre forskjellige tredemølleposisjoner.
Metode:
Studien har et overkrysningsdesign. Åtte transfemoralt amputerte av begge kjønn og med god helse deltok. Gjennomsnittsalder var 47 år (min: 22 - maks: 61). Oksygenforbruk og hjertefrekvens ble målt ved gange på selvvalgt hastighet. Subjektene gikk på en tredemølle i tre forskjellige posisjoner i randomisert rekkefølge: horisontal (posisjon l), 3 % tilt i sagittal plan (posisjon ll), 3 % tilt i sagittal og frontal plan (posisjon lll). Deskriptive data er gitt i median (min - maks) og forskjell er undersøkt ved bruk av Wilcoxon Sign Rank Test. Borg CR 10 skala ble benyttet for å undersøke deltakernes opplevde anstrengelse samt for å analysere eventuell korrelasjon med oksygenforbruk. Korrelasjon er undersøkt ved bruk av Spearman Rank Order test. Maksimal aerob kapasitet ble målt for å undersøke andelen av Vo2 /kg maks forbrukt ved gange på de tre tredemølleposisjonene.
Resultater:
Forskjell i oksygenforbruk mellom tredemølleposisjon l og lll viste en median på 21 %, (min: 9, 4 % - maks: 24, 8 %). Mellom posisjon l og ll var forskjellen i oksygenforbruk i median = 7,7 % (min: 2, 5 % -maks: 14 %). Dette ga en økning i oksygenforbruk på 13,9 prosentpoeng mellom posisjon ll og lll. P-verdien for differansen i oksygenforbruk mellom posisjon l-ll og l-lll var p=.012. P-verdien for differansen i opplevd anstrengelse mellom posisjonen l-ll og l-lll var p=.011. Korrelasjonen mellom opplevd anstrengelse og oksygenforbruk for den relative forskjellen mellom posisjon l-ll og l-lll var lav (r² = 12, 8 % og 9, 5 %). Testpersonene forbrukte i gjennomsnitt 61,4 % (min: 39 % - maks: 77 %) av deres Vo2/kg maks ved gange på tredemøllen i posisjon lll.
Konklusjon:
Transfemoralt amputerte med et høyt fysisk funksjonsnivå bruker signifikant mer energi når de går på en tredemølle moderat skråstilt i ett og to plan sammenlignet med gange på horisontalstilt tredemølle. De bruker også en stor andel av sin maksimale aerobe kapasitet ved gange på skråstilt underlag. Opplevd anstrengelse viste en signifikant økning mellom de tre tredemølle posisjonene, men det var en lav korrelasjon mellom subjektivt opplevd anstrengelse og de objektive dataene for oksygen forbruk.
ABSTRACT
Background and aim:
Walking on tilted pavements or in terrain with the highest level under the prosthetic foot is reported by transfemoral amputees as strenuous. This study investigates difference in energy expenditure and perceived exertion amongst transfemoral amputees walking on three different treadmill positions.
Method:
The study has a cross over design. Eight healthy unilateral transfemoral amputees of both sexes participated. The mean age was 47 years, (range 22 – 61). Difference in energy expenditure was measured by oxygen consumption and heart rate at self selected speed walking randomly on a treadmill in three positions; horizontal treadmill (position l), treadmill with 3 % tilt in sagittal plane, (position ll) and a treadmill with 3 % tilt in both sagittal and frontal plane (position lll). Descriptive statistics are given in median (min – max), and difference is investigated by use of Wilcoxon Sign Rank test. The Borg CR10 Scale was utilized to investigate the difference in perception of exertion in the test positions, and Spearman Rank order test was used to explore correlation with oxygen consumption. Maximal aerobic capacity (Vo2/kg max) was measured to investigate the proportion of maximal capacity utilized in the three experimental treadmill positions.
Results:
Difference in oxygen consumption between position l and lll was in median 21%, (range: 9, 4% - 24, 8%). Difference in oxygen consumption between position l and ll was in median 7, 7 %, (range: 2, 5% - 14, %). This gave a difference in oxygen consumption of 13, 9% point between position ll and lll. The p-value for relative difference between position l -ll and l -lll was p=.012. The p value for relative difference in perceived exertion between position l-ll and l-lll was found to be p=.011. The correlation between difference in perceived exertion and oxygen consumption for position l-ll and position l-lll was: r² = 9, 5 % and 12, 8%. The subjects utilized in median 61, 4% (range: 39 % - 77 %) of their VO2/kg max walking on a treadmill in position lll.
Conclusion:
Transfemoral amputees with high functional ability uses significantly more energy when walking on a tilted surface in on and two planes compared to a horizontal surface. The subjects’ perceived exertion showed a significant difference between gait on the three experimental positions of the treadmill. The correlation between perceived exertion and oxygen consumption was of low to medium strength. They also utilize a high proportion of their maximal aerobic capacity.
