Hensikt/formål
Studiens formål var å få innsikt i og forståelse for hvilke syn avdelingssykepleier har på implementeringen av pasientansvarlig sykepleie. Hva legger hun i begrepet og vil pasientansvarlig sykepleie få konsekvenser for hennes rolle som avdelingssykepleier?
Valg av metode
Kvalitativt, deskriptivt design ble valgt. Seks intervjuer av avdelingssykepleiere ble gjennomført ved to sykehus. Datasamlingen ble utført via kvalitativt intervju(Kvale, 1997, Fog, 2004)), Datamaterialet ble analysert etter en metode beskrevet og utviklet av
Kvale(1997, 2005).
Bakgrunn og ramme
Interessen for tema som resulterte i dette forskningsprosjektet, er knyttet til mine erfaringer som sykepleier gjennom 25 år, og tidligere undersøkelser som viste at avdelingssykepleier
innehadde en sentral rolle ved implementering av pasientansvarlig sykepleie(Gulbrandsen, 2002). I 1997 kom Steine-utvalget med en innstilling i NOU 1997:2 pasienten først! I følge utvalget
ville pasientansvarlig sykepleier sikre pasienten, og sørge for at helhetsperspektivet kunne bli bedre på sykehus. Avdelingssykepleierne på sengepost ble anmodet om å organisere sykepleietjenesten etter en modell og ledelsesfilosofi, omtalt som pasientansvarlig sykepleie.
Resultat
Respondentene hadde fått i oppdrag og implementere pasientansvarlig sykepleie på sine respektive sengeposter av ledelsen på sykehusene. Studien viser store variasjoner blant respondentene i hvordan de hadde lykkes med å implementere pasientansvarlig sykepleie.
Respondentene mente de hadde implementert pasientansvarlig sykepleie, og at dette var en modell som ga pasient og pårørende kontinuitet. Studien viser at sengepostene hadde gruppesykepleie med en sykepleier som fungerte som gruppeleder i en uke av gangen. Avdelingssykepleieren ønsket å få mer uformell og formell tid sammen med sykepleierne og flere hadde delegert den daglige driften av sengeposten til en sykepleier i stab. Det kunne være assisterende avdelingssykepleier eller gruppeleder. Studien viser at ingen av respondenten hadde implementert en rendyrket pasientansvarlig
sykepleie modell. Respondentene beskrev viktigheten av å ha høy sykepleiefaglig kompetanse i sengeposten og kontinuitet i sykepleiernes turnus.
Aim
The aim of the study was to gain insight into and understanding of how the ward sister views the implementation of patient responsible nursing care. The ward sister ` comprehension of the concept and if its` use will have consequences for her as ward sister.
Choice of method A qualitative, descriptive design was chosen. Six ward sisters at two hospitals were interviewed. Qualitative interviews were used to collect data, as described by Kvale (1997)
and Fog(2004). The data was analysed using a method developed and described by Kvale(1997, 2005).
Background and framework
My interest for this subject was aroused during my 25 years experience as a nurse, and an earlier study showed that a ward sister has a central role in the implementation of patient responsible nursing care(Gulbrandsen, 2002) The Steine-committee recommended, in NOU 1997:2. The patient first! that ward sisters organised nursing care on the wards after a model and leadership philosophy called patient responsible nursing care, as the committee considered this model would safeguard the patients best, as well as resulting in a more holistic approach in the hospitals.
Results
The respondents had been instructed, by the hospital management; to implement patient responsible nursing care on the ward. The study indicates some variation in how far the respondents had come in the implementation of patient responsible nursing care. The respondents meant they had implemented patient responsible nursing care, and that it was a model which secured continuity in care for patients and their families. The study indicates that the patient were organised in groups with one nurse designated as group-leader for each group
for a week at a time. The ward sister wanted both more formal and informal time with nurses and several of them had delegated the day to day running of the ward to a stab-nurse. This could be an assistant ward sister or a group-leader. The study shows that none of the respondents implemented a pure primary nursing model. The respondents described the importance of having a high level of nursing competence on the ward and continuity in the
nurses` rota.
