Sammendrag
Bakgrunn og hensikt:
Dagens sykehjem benyttes i stor grad som en varig boform i livets sluttfase. Forskning har identifisert aspekter ved omgivelsene inne i sykehjem, mens forskning fra norske forhold om de ytre omgivelsenes betydning er begrenset. Studiens hensikt var å bidra til økt kunnskap og forståelse om hvilken betydning natur og uteliv har for gamle.
Teoretisk forankring:
Studien bygger på miljøpsykologiske teorier til Kaplans og Ulrich om årsaker til naturens sterke virkning på menneskers helse og velvære.
Benner og Wrubels sykepleieteori utdyper velvære som mål for sykepleie, særlig for mennesker som er avhengige av andre. Det fokuseres på meningsfullhet i tillegg til helsefremmende og styrkende faktorer i omgivelsene. Sansehagens historikk og dens innhold presenteres avslutningsvis.
Metode:
Studien er kvalitativ og beskrivende. Datainnsamlingsmetodene var deltagende observasjon og kvalitativt intervju. Utvalget i observasjonsstudien bestod av ti brukere og intervjustudien omfattet fire mentalt klare i alderen 84-100 år. Observasjonsstudien foregikk i det ene sykehjemmets sansehage og intervjustudien fant sted på et annet sykehjem.
Resultater:
Funn fra begge datakildene viste det verdifulle i å komme på sporet av minnene. Faktorer relatert til uteliv i trygge omgivelser, skilte datakildene. Mulighetene for uteliv syntes å være mindre enn behovet og flere av de mentalt klare kom ikke ut når de ønsket. Informantenes glede over naturen ble nærværende i deres tilbakeblikk. De synes å ha mindre kontroll over eget hverdagsliv enn brukerne i sansehagen og opplevde å være prisgitt omgivelsene. Erfaringene deres knyttes til indre og ytre barrierer og blir kalt et dobbelt fangenskap. Observasjonsstudien indikerte stor grad av tilgjengelighet og trygghet for beboerne som har tilgang til en sansehage. Omgivelsene fremmet og innbød til aktiviteter med, - eller uten personale. Funn fra bruken av sansehagen viste mulig overføringsverdi til alle gamle.
Konklusjon:
Resultatene antyder at en sansehage har betydning for de gamles velvære. Et helsefremmende perspektiv i omgivelsene forebygger trolig passivitet og ressursatrofi. Positiv sammenheng mellom natur og helse søkes bevisstgjort i sykepleiefaget. Regelverk som sikrer tilrettelegging for naturkontakt og uteliv for alle gamle i sykehjem bør etableres. Tverrfaglig samarbeid med aktuelle miljøer utfordres for å utnytte eksisterende kompetanse som kan bidra til kvalitetsutvikling i eldreomsorgen.

Access to nature and outdoor gardens in the care of the elderly, - luxury or necessity?
Healing Gardens importance for the elderly in nursing homes.
Purpose:
Nursing homes of today are most often a permanent form on living at the end of ones life. Research has identified definite aspects of the indoor environment. However, in Norway, the outdoor environment has received little attention. The purpose of this study was to increase the knowledge and understanding about the positive input that the natural environment and the outdoors can have on the elderly.
Literature Review:
This study is built upon the environmental-psychologist theories of the Kaplans and Ulrich. They support the strong effect that nature has upon humans health and well-being. Benner and Wrubels nursing theory expounds upon the parameter of well-being for nursing. Especially for people who are dependent on others help. The focus is on meaningful, health-promoting and health-strengthening factors in our natural environment. The history of healing gardens and it`s content conclude this section.
Method:
This study is qualitative and descriptive. The data collecting methods were participating observation and qualitative interview. The subject selection in the observational study included ten elderly (dement) nursing home patients. In the interview group were four mentally alert subject in the age range 84-100 years. The observational study was conducted in a healing garden in direct connection with a nursing home. The interviews were held at another nursing home.
Results:
The findings from both the data sources (study groups) showed the importance of finding a path to forgotten and lost memories. Factors related to the outdoor environment in secure surroundings separated the two data sources. The possibility of visiting an outdoor environment seemed to be less than the need and desire. Many of the mentally alert elderly did not get outside as often as they had wished. The interviewed subject expressed joy over what nature had meant for them when they reflected back in time. They seemed to have less control over their everyday lives than the subjects in the observational study that could visit the healing garden whenever they wished. They were dependent upon their surrounding environment. The experience is connected to internal and external barriers and is termed: double imprisonment. The observational study indicated a large degree of access and security for the elderly that had healing gardens. The surroundings were conducive and invited to outdoor activities either with or without the nursing staff. These findings of the elderly peoples use of healing gardens showed possible values that could be generally transferred to the elderly.
Conclusion:
The results imply that a healing garden has value for well-being of elderly people in nursing homes. A health promoting perspective in the environment prevents passitivity and atrophy of mental and physical resources. The awareness of the positive connection between nature and health is attempted brought to mind in the field of nursing. Laws and regulations that secure the development and implementation of healing gardens in nursing homes should be established. Multidisciplinary cooperation is encouraged to take advantage of the existing knowledge that can be used to improve the quality of elderly nursing care.
