Abstract
Hovedhensikten med studien var å undersøke og beskrive pasientens opplevelser i forbindelse med respiratorbehandling. Opplevelsene jeg ønsket å få tak i er relatert til stressorene; over- og understimulering, fremmedgjøring, smerter og ubehag. Disse opplevelsene hos pasienten ser jeg opp mot hva sykepleierne gjør, og hva som eventuelt kan endres.
Bakgrunn for studien:
Etter mange års praksis som intensivsykepleier, var jeg opptatt av hvordan pasienter egentlig hadde det mens de gjennomgikk behandling på respirator. Dette gjorde meg nysgjerrig. Det var få empiriske studier som var gjort på emnet i 1985. Jeg ønsket derfor å ta utgangspunkt i pasientens beskrivelse av sin opplevelse.
Den teoretiske fase innledes med en utdyping av sykepleiefaglig ståsted og viktige begrep for studien som mestring og stress.
Feltfasen er en fenomenologisk - hermeneutisk intervjuundersøkelse av 11 voksne pasienter som har gjennomgått respiratorbehandling. Fenomen som er viktige i forbindelse med høyteknologisk behandling så som over - og understimulering, fremmedgjøring, smerter og ubehag er gjenstansområder for intervju.
Opplevelsene drøftes utfra valgte teoretiske begrep som stress og mestring.
I den avsluttende fase diskuterer jeg konsekvensen av de funn jeg har gjort utfra tre av Orem's hjelpemetoder; handle for, støtte og skape terapeutisk miljø. Jeg ser konsekvensen for sykepleie før, under og etter behandling med respirator.
The main purpose of this study was to find out and describe the patient's experiences in connection with treatment on respirator. The experiences I wanted to get hold of are related to the stressors; sensory overload - and deprvation alientation, pain and discomfort. I would like to see the patient's experiences in the light of what the nurses do and what they can do better.
Background for the study:
After many years experience as an intensive care nurse, I was occupied with the thought of how patients really are while they go through treatment on respirator. This made me curious. Few empiric studies have been made on this subject until 1985. I therefore wanted to start out with the patient's description of her/his experience.
The theoretical phase starts by presentin my nursing platform and important ideas for the study as stress and coping.
The fieldwork phase is a phenomenological - hermeneutical examination based on interviews of 11 adult patients who have gone through treatment on respirator. Phenomenoms that are important in connection with high technological treatment like sensory oveload- and deprvation, alientatin, pains and discomfort are the subjects for the interviews.
The experiences will be discussed from chosen theoretical ideas like stress and coping.
In the final phase I discuss the consequence of the results I have made from three of Orem's aid methods; acting for or duing unanther, supporting another and providing an envirement that prmotes personal development. I see the consequence for nursing before, under and after treatment with respirator.
