Abstract
Formål: Å utforske og beskrive erfaringene til sykepleiere fra omsorg for pasienter i posttraumatisk amnetisk fase (PTA-fasen).
Teoretisk perspektiv: En fenomenologisk tilnærming til erfaringene sykepleierne har fra møter med pasienter i posttraumatisk amnetisk fase. Rådende litteratur ligger til grunn for utvikling av studiens forskningsspørsmål. Forskningsspørsmålene retter seg mot hva som kjennetegner de gode og mindre gode omsorgssituasjonene i møter med pasienter i PTA-fasen, og hvordan sykepleierne bruker seg selv i omsorgen for pasienter i PTA-fasen.
Metode: Studien er inspirert av den fenomenologiske metode, og har et utforskende og beskrivende design. Det er intervjuet åtte sykepleiere, og fortellinger fra erfaringene utgjør studiens data. Utvalget er basert på en frivillig deltakelse, med krav om minimum ett års erfaring fra arbeid med pasienter i PTA-fasen. Analysen er gjennomført ved å følge Giorgis fenomenologiske metode (1985).
Resultater: Studiens vitenskapelige essens er benevnt den semipermeable omsorgen, som symboliserer hvordan sykepleierne skjermer pasientene fra omgivelsene, ved å lukke ute sterke stimuli, og åpne for en varsom tilnærming til omverdenen på en måte som bidrar til at pasienten kan orientere seg, og oppleve å være forankret med sin kropp og i sine tanker. Fenomenet sykepleie til pasienter i PTA-fasen er presentert igjennom seks meningsemner som sammen viser hvordan sykepleierne erfarer omsorgen for pasienten; hvordan de opplever sin egen rolle og ansvar, pasientenes atferd og ivaretakelse av de pårørende.
Konklusjon: Skjerming er grunnleggende i omsorgen for PTA-pasienter. Det er av avgjørende betydning at sykepleierne foretar grundige vurderinger ved iverksetting av tiltak, av faglig, skjønnsmessig og etisk karakter. Forskning videre bør fokusere på fortsatt utvikling av en felles forståelse av fenomenet PTA, og bør inneha pasientenes og de pårørendes perspektiver. Det bør også gjøres en sammenlikning av behandlingstilbudet de ulike foretakene i Norge gir pasienter i PTA-fasen, med fokus på varighet og effekt av behandling.

A study of nurses experiences from the care of patients with post-traumatic amnesia
Purpose: To explore and describe nurse's experiences from the care of patients with post-traumatic amnesia (PTA).
Theoretical perspective: A phenomenological approach of nurse's experiences from their work with patients with PTA. Current literature has been explored in advance of developing the study's inquiry. The study investigates characteristics of positive caring situations, as well as the negative. It also takes into consideration how nurses use one's self in the care of these patients.
Method: The study is inspired by the phenomenological method, and has an explorative descriptive design. Eight nurses were interviewed, and their narratives constitute the data of the study. The election of participants is based on voluntary participation, and a minimum of one year of experience from working with patients with PTA. The analysis is based on Giorgis phenomenological method (1985).
Results: The essential characteristics of the phenomenon has been called semi permeable care, which symbolizes how the nurses protect the patients by physically guiding and modifying the surroundings, i.e. closing off distracting stimuli and cautiously reintroduce their surroundings in a way that helps the patient to cognitively orient his body and mind. The phenomenon nursing care of patients with PTA is presented through six constituents: how the nurses experience this particular type of care, their role and the responsibilities, the behavior of the patients and the involvement of the patient's relatives.
Conclusion: Isolation and protection are paramount in the care of the patients with PTA. It is of great importance that the nurses take into consideration expert knowledge, personal judgment and ethical aspects when they provide this type of care. Further studies are needed to develop a global understanding the phenomenon of PTA, and should also include the perspectives of the patients and their relatives. It is also important to look at how different hospital facilities in Norway treat and care for this patient group, focusing on duration of PTA and effect of treatment outcomes.
