Abstract
Bakgrunn: Dette er en systematisk oversiktsartikkel over randomisert kontrollerte studier (RCT) som ser på behandling av midtporsjons Akillestendinopati med autolog blodinjeksjon i kombinasjon med eksentrisk styrketrening. Hensikten med studien er å vurdere kvaliteten av relevante studier ved The Grading of Recommendations Assessment Development and Evaluation (GRADE)-systemet og undersøke om man kan anbefale autolog blodinjeksjon i kombinasjon med eksentrisk styrketrening som behandling for midtporsjons Akillestendinopati. Metode: Det ble utført et systematisk litteratursøk i søkemotoren PubMed med friteksten [Achilles Tendinopathy ((PRP) or (Autologous blood injection))] frem til 16. februar 2014 og filtrene RCT, engelsk og human ble benyttet. Inklusjonskriteriene var at det skulle være en RCT med autolog blodinjeksjon i kombinasjon med eksentrisk trening som intervensjon på personer med midtporsjons Akillestendinopati. Som kontroll skulle det vært benyttet eksentrisk styrketrening og utfallet skulle være målt med The Victorian Institute of Sports Assessment – Achilles Questionnaire (VISA-A). Resultat: Totalt tre studier oppfylte kriteriene for inklusjon og materialet i disse utgjorde totalt 140 personer og 147 Akillessener. Samtlige studier ble vurdert til å være av moderat kvalitet etter GRADE-systemet. Intervensjons- og kontrollgruppene fikk begge en økning i VISA-A i løpet av oppfølgingstiden, men det ble ikke funnet signifikante forskjeller mellom de to ulike gruppene. Konklusjon: Det finnes i dag ikke dokumentasjon for å behandle midtporsjons Akillestendinopati med autolog blodinjeksjon i kombinasjon med eksentrisk styrketrening fremfor eksentrisk styrketrening alene.
Background: The objective of this study was to conduct a systematic review of clinical outcome following autologous blood injection in combination with eccentric strength training for the treatment of midportion Achilles Tendinopathy. The aim was further to evaluate the quality of studies using The Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach and based on this give a recommendation whether autologous blood injection in combination with eccentric strength training should be used or not. Method: A systematic search of PubMed was performed until 16. February 2014 using the terms [Achilles Tendinopathy ((PRP) OR (Autologous blood injection))] and the filters RCT, English and Human. The inclusion criteria were randomized controlled trial (RCT) with autologous blood injection in combination with eccentric strength training for the treatment of midportion Achilles Tendinopathy in human. The control group had to conduct eccentric strength training as their treatment and the Victorian Institute of Sports Assessment – Achilles Questionnaire (VISA-A) had to been used for the measure of the outcome. Result: 3 studies fit the inclusion criteria for our review, constituting 140 patients and 147 Achilles tendons. All the included studies were evaluated as moderate quality studies using the GRADE approach. Both the intervention and control group had increased VISA-A score from baseline, but no significant difference between the groups were found at any point. Conclusion: Today, no evidence favors treatment of midportion Achilles Tendinopathy with autologous blood injection in combination with eccentric strength training over eccentric strength training alone.
