Abstract
Bakgrunn Teorien i kognitiv atferdsterapi (KAT) er at automatiske tankemønstre styrer følelser, kroppslige reaksjoner og atferd. Unge mennesker med psykiske helseplager har trolig mindre fastlåste tankemønstre enn voksne. Hovedproblemstillingen i dette arbeidet er: opplever unge pasienter å ha fått hjelp av å gjennomgå kognitiv atferdsterapeutisk behandling? Metode For å belyse problemstillingen har jeg benyttet en kvalitativ metode. Jeg har gjort semistrukturerte intervjuer av tre unge (mellom 14-20 år) pasienter, to jenter og en gutt. Materialet presenteres i denne prosjektoppgaven og eventuelt senere som en artikkel i et medisinsk tidsskrift. Derfor har jeg søkt og fått studien godkjent av REK (regionale etiske komité). Resultater 1.Tenk konstruktivt. Andres negative tanker om meg er ikke viktig. De unge pasientene har lært å tenke konstruktivt og å fokusere på realistiske alternativer i stedet for det negative i ulike situasjoner. De har også fått et mer hensiktsmessig bilde av andres tanker om dem. 2. Bevisst forventning til mestring og relasjoner. Etter behandlingen fikk deltakerne i studien et mer bevisst forhold til mestring og ble mer selektive i å velge venner som deler ønsket om å fremme mestring. 3. Bedre selvtillit, takler mer. Deltakerne i studien rapporterer å ha fått bedre selvtillit, bedre livskvalitet og økt evne til å mestre stressende situasjoner etter gjennomgått behandling. 4. Ta ansvar selv. Gjennom terapien fortalte de unge pasientene at de lærte selv måtte ta ansvar for å få en bedre livssituasjon. De lærte at det gjelder å møte utfordringer i livet med aktive valg og handlinger. Behandlingen ga dem et redskap som de kan bruke når de møter disse utfordringene, men de må selv aktivt bruke det. 5. Overføre på de rundt seg. KAT oppleves som positivt og gir effekt ved behandling av unge pasienter. Behandlingen er så effektiv at de unge pasientene ønsker å videreføre sine erfaringer og sin lærdom fra terapien til sine nærmeste. Diskusjon Unge pasienters opplevelse av KAT er lite beskrevet. Gjennom en kvalitativ studie har vi fått innsikt i hvordan tre unge har opplevd å gjennomgå kognitiv terapi. Antallet intervjuede er lite. Å samle kunnskap med utgangspunkt i brukeren, unge mennesker, vil være med på å bedre kvaliteten i tilbudet til denne aldersgruppen.
Background The theory in cognitive-behavioral therapy (CBT) is that feelings, physical reactions and behavior are results of automatic thoughts. The thinking patterns of young people with psychological health issues are probably easier to affect than adults . The main topic in this work is: does young people experience that CBT helps them in recovering from psychological health problems? Methods We have done a qualitative study to enlighten the topic. The study is based on three interviews with young patients (between 14-20 years), one boy and two girls. The main findings from the material are presented in this work and eventually later as an article in a medical journal. I applied to REK (regional etisk komité) for permission to do the study. REK approved the study. Results 1.Think constructively. Negative thoughts from others are not important. The young patients have learned to think constructively and to focus on realistic alternatives in stead of the negative aspects of different situations. They have also got a more realistic picture of others thoughts about them. 2. Well-considered expectations about relations to others and mastering challenges. By going through the treatment, the participants in the study got more realistic expectations about mastering challenges in life. They also became more selective in their priorities of who to spend time with. They learned to choose the ones who supports them and helps them to deal with their problems. 3.Better self-confidence, better to handle challenges. The young patients in the study reports that they by going trough the treatment have got better self-confidence and better quality of life. They have also increased their ability to master stressful situations. 4. Be responsible. The young patients learned that they have to take the responsibility for their own lives if they want to get a better life-quality. The treatment taught them to meet challenges in life with well-considered choices and actions. The treatment gave them tools which can help them in dealing with challenges in their daily lives, but the patients have through the treatment realized that they have to use the tools themselves. 5. To transfer their knowledge to friends and family. CBT is a positive experience and is effective in the treatment of the young patients in this study. The treatment is so effective that the participants in the study want to transfer their experiences and knowledge to the ones in their close proximity. Discussion Young patients experience of going through CBT is a topic that is not well explored. By doing this qualitative study, we have got knowledge about how three young patients experienced going through CBT. Even though it s a small number of participants, this study contributes to new knowledge about the topic. Collecting knowledge based on the patients points of views will better the quality of the treatment that we offer for patients in this group of age.
