Kunnskapsbasert praksis angår alle, også ergoterapeuter : En kvalitativ studie av ergoterapeuters forståelse av kunnskapsbsaert praksis.

Abstract

Formål: Hensikten med denne studien er å belyse hvordan ergoterapeuter forstår og utvikler kunnskapsbasert praksis, og derigjennom å bidra til refleksjon og diskusjon av kunnskapsbasert praksis i ergoterapimiljøet i Norge. Teoretisk forankring: Studien har et sosiologisk vitenskapsteoretisk perspektiv inspirert av Berger og Luckmann, og en konstruksjonistisk diskursanalytisk tilnærming. Metode: Prosjektet har en kvalitativ tilnærming. Datamaterialet er skapt gjennom fire semistrukturerte individuelle intervju og et fokusgruppeintervju med fire deltagere. Analysearbeidet har en diskursanalytisk tilnærming.

Resultater: I materialet trådte det frem fire diskurser, som informantene fordelte seg mellom: Den teoriforankrede, den forskningsbaserte, den relasjonelle og den kliniske diskursen. Hvordan ergoterapeuter forstår og utvikler kunnskapsbasert praksis er avhengig av forankring i diskurs og kontekst. Felles for forståelsen for alle ergoterapeutene er at de inkluderer anvendelse av forskningskunnskap fra ”effektforskning” for planlegging av tiltak til enkeltpasienter eller pasientgrupper. Informantene har utviklet sin forståelse av kunnskapsbasert praksis gjennom videreutdanning, internundervisning, litteraturstudier og i kontakt med lokale, nasjonale og internasjonale miljøer. Alle snakker om forskning og forskningskunnskap når de skal se på forholdet mellom ergoterapi og kunnskapsbasert praksis og når de skal beskrive hvordan de har integrert det i egen praksis. Noen anvender kunnskapsbasert praksis i klinisk praksis, mens andre er opptatt av å spre kunnskap om kunnskapsbasert praksis og hvordan ergoterapeuter kan finne, vurdere og anvende forskningskunnskap. Noen utfordrer også kunnskapssenterets konstruksjon av kunnskapsbasert praksis og oversettelsen av evidensbasert praksis til kunnskapsbasert praksis.

Konklusjon: Det eksisterer ingen felles forståelse av kunnskapsbasert praksis blant norske ergoterapeuter. De anvender det i sin praksis ut fra sin forståelse. Forståelsen utvikles i diskurs og kontekst. Informantene utfordrer norske ergoterapeuter til debatt og begrepsavklaring. Informantene utfordrer til fagutvikling, forskning og anvendelse av forskningskunnskap, og til å gjøre forskningskunnskapen tilgjengelig, for å utvikle behandlingstilbudet til pasientene.

Summary

Purpose: The purpose of this study is to elucidate how occupational therapists understand and develop evidence – based practice (kunnskapsbasert praksis), and in that way contribute to reflection and discussion on evidence – based practice among Norwegian occupational therapists.

Theory base: The study has a sociological theoretical perspective inspired by Berger and Luckmann and a constructionistic discourse analytic approach. Theory on evidence – based practice. Theory on Occupational Therapy.

Method: The project has a qualitative approach. The empirical data has been created through four semistructured individual interviews and one focusgroupinterview with four participants. The analysis is done with a discourse analytic approach.

Results: There appeared four discourses in the data, which the informants found their places in, the theory – based, the research – based, the relational and the clinical discourses. How occupational therapists understand and develop evidence - based practice depends on discourse and context. One common element in their understanding is that they include the use of knowledge from quantitative research in the planning of treatment for individual patients and groups of patients. The informants have developed their understanding of evidence – based practice through studies, education at work and literature studies, in local, national and international contexts. All of them talk about research and knowledge from research when they consider the relationship between occupational therapy and evidence – based practice and when they describe how they have applied it into their work. Some of them have applied evidence - based practice into clinical practice, while others focus on sharing evidence –based practice and how occupational therapists can use knowledge from research. Some of them question “Kunnskapssenteret”’s construction of evidence – based practice and the translation to “kunnskapsbasert praksis”(“knowledge - based practice”).

Conclusion: There is no common understanding of evidence – based practice among Norwegian occupational therapists. They apply it into their practice as they understand it. The understanding is developed in discourse and context. The informants give occupational therapists challenges: To discuss and define evidence – based practice, to develop occupational therapy and to research, to apply knowledge from research into their practice, and to make the knowledge from research accessible and available for the development of the treatment for the patients.
