Sjette avdeling : Et utforskende møte mellom fortid og nåtid med utgangspunkt i Anton Tsjekhovs skjønnlitterære tekst Palata No.6 fra 1892

Abstract

Den russiske forfatteren Anton Tsjekhovs langnovelle Sjette avdeling (1892) har en sentral plass i denne studien. Tsjekhovs tekst kaster lys over en psykiatrisk praksis på det stedet i historien hvor de psykiatriske avdelingene ble innlemmet i somatiske sykehus.

Hensikten med studien er å utfordre dagens forståelse av det psykiske helsevernet ved å utforske møtet mellom denne forhistoriske praksisen og dagens praksis. Oppgaven åpner dermed et spenningsfelt mellom fiksjon og erfaring, fortid og nåtid, kontinuitet og diskontinuitet. For å utforske dette møtet er det foretatt nærlesning av teksten pluss samtaler med en kvalifisert medleser som både har erfaringer som pasient og ansatt i dagens psykiske helsevern.

Masteroppgaven har en diskursorientert forankring og er inspirert av den franske filosofen og samfunnstenkeren Michel Foucaults arbeider hvor han trekker forbindelseslinjer mellom nåtidens og fortiden psykiatriske praksiser.

Oppgavens analyse er også inspirert av Foucaults maktanalyser, hvor individets mulighetsrom og frigjøringsmuligheter i aktuelle diskurser står sentralt. Studien peker mot klare diskursive likhetstrekk mellom fortidens og nåtidens praksis, selv om de ytre rammebetingelsene er endret. Kontinuiteten er tydelig ved at fagdisiplinenes diskurs fortsatt har en definisjonsmakt over pasientenes mulighetsrom inne i institusjonen. Studien avdekker ulike reguleringsmekanismer som trer i kraft for å opprettholde posisjoneringene som behandler og pasient. Analysen viser hvordan utestengningsmekanismer aktiveres innad på behandlersiden hvis ansatte utfordrer fagdiskursens regler og handlemåter. Videre avdekker analysen at det er vanskelig å posisjonere pasienterfaringene innenfor fagdisiplinenes diskurs. En forutsetning for å posisjonere erfaringene ser ut til å være å ta steget ut av institusjonen til et utvendighetsrom. Dette er tilsynelatende et brudd med den forhistoriske praksisen. Analyseobjektene fra fagdisiplinenes diskurs gis i vår tid posisjoneringsmulighet i en diskurs som utfordrer fagdisiplinenes diskurs fra utsiden. Dette kan forstås som en frigjøringsmulighet hvor diskursene kan komme i reell forhandlingsposisjon som kan gi retning for ny begrepsdannelse og kunnskapsutvikling. Studien peker imidlertid mot at utvendighetsrommets iscenesetting av de individuelle fortellingene kan spores tilbake i psykiatriens historie, hvor det er en lang tradisjon for at analyseobjektenes fortellinger formidles og tas i bruk som bekjennelser.

Abstract

The Russian author Anton Chekhov’s novella Ward Six (1892) has a central place in this study. Chekhov’s text sheds light on psychiatric practice at the time when psychiatric wards were first incorporated into somatic hospitals.

The aim of this study is to challenge the current understanding of psychiatric health care by exploring the meeting between this historical and current practice. In so doing, the study explores the juxtaposition between fiction and experience, past and present, continuity and discontinuity. To explore this meeting, the study combines close reading of Chekhov’s text and conversations with a qualified co-reader with experience as both patient and employee in today’s psychiatric health care system.

This master thesis is based on a discourse-oriented approach and inspired by the works of French philosopher and social critic Michel Foucault, where he draws parallels between present and past psychiatric practises. In addition, the analysis in this study is inspired by Foucault’s power analyses, where spaces of possibilities and liberation opportunities in actual discourses are key.

The study points to clear discoursive similarities between past and present practice, even though the external premises have changed. Continuity is apparent from the fact that professional discourse has retained definition power over the space of possibilities of the patient within the institution. The study reveals a number of regulative mechanisms that maintain the positioning as carer and patient. The analysis shows how exclusion mechanisms are activated among carers if employees challenge the rules and modes of action of the professional discourse. Furthermore, the analysis reveals the difficulty in positioning patient experiences within the professional discourse. A precondition for the positioning of experiences appears to be stepping out of the institution to an external space. This appears to be a break with past practice. In our time, the objects of analysis from the professional discourse are given positioning opportunity in a discourse that challenges the professional discourse from the outside. This can be regarded as a liberation opportunity, whereby discourses may enter into a real negotiation position which can provide direction for the creation of new concepts and knowledge development. However, this study indicates that staging individual stories in external spaces can be traced back through the history of psychiatry, where there is a long tradition for individual stories to be presented and used as confessions.
