Abstract

Tittel og undertittel: Forestillingene som brast. Sykepleierstudenters erfaringer med relasjonsarbeid i akuttpsykiatrisk praksis.

Formål: Studiens hensikt er å beskrive sykepleierstudenters erfaringer med å komme i kontakt med og etablere en relasjon til pasienter innlagt i akuttpsykiatri i praksisperioden på 8 uker.

Litteraturgjennomgang: Studien presenterer litteratur og forskning om sykepleier - pasient - relasjonens innhold, kvaliteter og betydning innen psykiatrisk sykepleie. Det presenteres også forskning om sykepleierstudenters holdninger til pasientarbeid i praksis i psykisk helsearbeid generelt. Det viser seg at sykepleierstudenter i mange tilfeller har negative holdninger til det psykiatriske fagfeltet, og er spesielt engstelige for å være på en akuttpsykiatrisk avdeling. Dette, i tillegg til akuttpsykiatriens forutsetninger som kort liggetid og svært sårbare pasienter, påvirker studentenes muligheter for å komme i kontakt med og etablere en relasjon til pasientene i akuttpsykiatrisk praksis.

Metode: Studien har kvalitativt, beskrivende design. Det ble gjennomført dybdeintervju med syv sykepleierstudenter i akuttpsykiatrisk praksis. Data er analysert ved bruk av kvalitativ tekstkondensering.

Funn: Funnene viser at sykepleierstudentens erfaringer med pasientene og kjennskap til behandlingen i akuttpsykiatrien vendte negative holdninger, forestillinger og engstelse til interesse, engasjement og forståelse for psykisk sykdom og psykiatrisk sykepleie. Funnene viser også at studentene erfarte å utvikle en rekke kommunikative ferdigheter, ble tryggere på seg selv i samhandling med pasientene, og gjorde seg opp meninger og tanker om viktige forutsetninger for en sykepleier – pasient - relasjon.

Konklusjon: Studien frembringer utdypede beskrivelser av hvilke erfaringer med pasientarbeid som var betydningsfulle for sykepleierstudenters forståelse av sykepleier – pasient - relasjonen i praksis. Funnene bidrar til kunnskap om hvilke situasjoner i praksis som var verdifulle for deres utvikling, og er et innspill i arbeidet med å tilrettelegge for en praksisperiode for sykepleierstudenter som bidrar til innsikt i psykiatrisk sykepleie, pasientenes behov og behandling av psykisk sykdom.

Abstract: Purpose: The purpose of this study is to describe the experiences that nursing students experience when they get in contact with and establishes relations to patients admitted to an acute psychiatric ward, during their eight week period of clinical practice.

Literature: The study presents literature and research results regarding nurse-patient-relations, qualities and meaning within psychiatric nursing. Research regarding nursing student`s attitudes towards working with patients during their period of clinical practice in mental health nursing is also represented. It appears that nursing students in many cases have negative attitudes to the psychiatric field, and they are especially anxious about being on an acute psychiatric ward. This, in addition to common features in the acute psychiatric wards, such as short in-patient stay and very vulnerable patients, affects the students possibilities to get in contact with, and establish relations to patients during such a relatively limited time.

Method: This study used a qualitative and describing design. In-depth interviews were carried out with seven nursing students towards the end of their clinical practice on an acute psychiatric ward. The data were analysed from a qualitative perspective by abstracting concepts from the in-depth interviews based on the condensation of text.

Findings: The results of this study show that the student`s clinical experiences with psychiatric patients and the knowledge they experience from being on an acute psychiatric ward turned around their negative attitudes, imaginations and worries to interest, engagement and understanding of psychiatric illness an psychiatric care. The results also show that the students developed different communication skills, got more confident when interacting with patients and made up independent opinions and thoughts about important conditions for a nurse-patient-relation.

Conclusion: The study brings forward rich descriptions of which experiences of patientwork that were meaningful for the nursing students comprehension of nurse-patient-relations in clinical practice. The findings contributes to knowledge about which situations that were valuable for the nursing students own growth, and might be helpful as guidance when organizing/preparing for the student´s clinical practice that might contributes to the students development and insight in psychiatric nursing, the patients needs and treatment of psychiatric illness/disease.
