Psykiatrisk sykepleiers rolle og funksjon i dagens tverrfaglige miljøterapi

Abstract

Formålet med denne eksplorative, deskriptive studien er primært å belyse rollen til psykiatriske sykepleiere i dagens tverrfaglige miljøterapeutiske team, dernest å belyse hvilken berettigelse psykiatrisk sykepleier har i tverrfaglig miljøterapi, og endelig å belyse hvilke utfordringer sykepleien stilles overfor i en eventuell videreutvikling av tverrfaglig samarbeid.

Den sykepleieteoretiske forankring ligger i Hildegard Peplaus arbeider om psykiatrisk sykepleie og i Kari Martinsens omsorgsteori, og studien ser på hvordan deler av disse forfatteres teori kan forstås innen en miljøterapeutisk kontekst. Sykepleievitenskapens teoretiske struktur blir sett i lys av Suzie H. Kims vitenskapshierarki og metaparadigmenivåets fire domener. Studiens funn plasseres i praksis- og omgivelsesdomenet. Som metode brukes kvalitativt forskningsintervju av fire sykepleiere og fire miljøterapeuter på en psykiatrisk langtidspost.. Analysen er forankret i et fenomenologisk-hermeneutisk forståelsesperspektiv.

Resultatene viser at sykepleierne opplever et tungt faglig ansvar for sine pasienter og ønsker å dele dette med miljøteraputene. Tverrfaglig miljøterapi oppleves fruktbart for begge parter. Både sykepleiere og miljøterapeuter har problemer med definisjon av begrepet miljøterapi. Sykepleieren tilpasser seg teamet hvor alle yrkesgrupper stort sett utfører samme funksjoner. Sykepleierrollen blir utydelig med manglende samsvar mellom sykepleieteori og praksis. Enkelte sykepleiere uttrykker en gryende bekymring for situasjonen.

Studiens funn indikerer at personalet bør arbeide med klargjøring av det ideologiske fundament og de praktiske implikasjoner for å forbedre metoden. Sykepleierne bør definere og formidle sin vitenskapstilhørighet og sitt sykepleieperspektiv, definere sykepleierens spesifikke oppgaver og tilegne seg kunnskap om de øvrige yrkesgrupper.

The purpose of this exploratory, descriptive study is, first, to explore the role of psychiatric nurses in today's interdisciplinary team, then to illuminate the legitimacy of the psychiatric nurse in interdisciplinary milieu therapy, and finally, to present challenges facing psychiatric nurses in the further development of interdisciplinary teamwork.

The theoretical frame of reference is based on Hildegard Peplau's theory of psychiatric nursing and Kari Martinsen's theory of nursing care. The study discusses how various elements of these theories can be understood within the context of milieu therapy. The theoretical structure of nursing science is viewed from Suzie H. Kim's hierarchy of science and the four domains of the metaparadigmatic level. The study findings are connected to the practice and environment domain.

The method used is the qualitative research interview, grounded on a phenomenological and hermeneutic understanding. Four nurses and four milieu therapists, working on a hospital ward for long term patients, participated in the study.

The findings show that the nurses experience the professional responsibility for the patients as heavy, and they wish to share the responsibility with the milieu therapists. Interdisciplinary milieu therapy is regarded as fruitful by both professional groups. However, defining the concept of milieu therapy is difficult for the respondents. The nurses have adapted to the team in which all team members in principle carry out the same functions. Thus, the consequences for the nurses are that their role has become vague resulting in lack of consistency between nursing theory and practice. This situation gives rise to concern for some of the nurse respondents.

The study findings indicate that the ideological basis and the practical implications for milieu therapy should be clarified in order to refine the method. The nurses should also critically analyse their role and function - and articulate their professional perspective - and also acquire solid knowledge of the other professions on the ward.
