Balanse mellom avhengighet og uavhengighet hos langtidspasienter i sykehjem

Abstract

Problemstilling: Hvilke oppfatninger og erfaringer har sykepleiere om hvordan balanse mellom avhengighet og uavhengighet blir ivaretatt hos langtidspasienter i sykehjem?

Underspørsmål: 1) Hvilke kunnskap gir sykepleiere til kjenne om dette emne? 2) Hvilke faktorer i omgivelsene fremmer eller hindrer denne balansen?

Den overordnede hensikten med den teoretiske delen av studien er å avklare nærmere en balanse mellom avhengighet og uavhengighet hos langtidspasienter i sykehjem, og å se på hvilke konsekvenser det kan ha for sykepleieutøvelsen. Hensikten med den empiriske undersøkelsen er å kartlegge sykepleieres oppfatning og erfaring med å ivareta en slik balanse hos denne pasientgruppen.

Studien har et beskrivende design, metoden er kvalitativ med fenomenologisk tilnærming. Det foretaes et semistrukturert intervju av syv sykepleiere med direkte ansvar for sykepleien til en gruppe langtidspasienter i sykehjem. Data analyseres ved hjelp av Giorgis fenomenologiske metode. Studiens teoretiske rammeverk bygger på Kari Martinsens omsorgsteori, Martin E.P. Seligmans teori om lært hjelpeløshet og Margret M. Baltes modell om lært avhengighet.

Funn fordelte seg i to hovedområder som synliggjør essensen i dette studiet og det er pasientenes atferd og personalets atferd. Pasientenes atferd blir beskrevet som passiv. Noen få unntak blir beskrevet å vise personlig vekst etter de har kommet til sykehjemmet. Det blir beskrevet at pasientene ber om hjelp til oppgaver de er i stand til å utføre selv, fordi det er eneste måten de får oppmerksomhet og sosial kontakt med personalet.

Personalets atferd og den betydningen det har for å ivareta balanse mellom avhengighet og uavhengighet ble beskrevet ut fra følgende underkategorier: Personalets oppfatning av avhengighet og uavhengighet, faglig vurdering av pasienten, målsetting og verdigrunnlag, personalets atferd overfor hverandre, personalets egne behov, oppgaveorientert eller individorientert sykepleie, kunnskapsgrunnlag, holdninger og stimuleringstiltak.

Konklusjon: For å kunne ivareta balanse mellom avhengighet og uavhengighet hos langtidspasienter i sykehjem må personalet modifisere egen atferd, og tilegne seg mer teoretisk kunnskap slik at de kan differensiere mellom ulike typer avhengighet. De vil da kunne vurdere og imøtekomme pasientenes reelle behov for hjelp, på bakgrunn av pasientenes egne behov og ressurser.

Principal Question: What conceptions and experiences do the nurses have concerning how balance between dependency and independency is taken care of in long-term patients who have been admitted to nursing homes?

Subsidiary questions: 1) What knowledge do nurses have about this subject? 2) What factors in the environment promote or prevent this balance?

The primary purpose of the theoretical part of the study is to take a closer look at this balance, and to examine the consequences this may have for the practising of nursing. The purpose of the empirical study is to clarify the nurses' conceptions and experiences in maintaining a balance between the dependency and independency in this group of patients.

The study has a descriptive design; the method is qualitative with a phenomenological approach. A semi-structural interview is undertaken of seven nurses who have a direct responsibility for the nursing of a group of long-term patients in a nursing home. Data is analysed using Giorgis phenomenological method. The theoretical framework for the study is based on Kari Martinsens Theory of Care, Martin E.P. Seligmans Theory of Learned Helplessness and Margret M. Baltes Model of Learned Dependency.

The study's results can be divided into two main fields that clarify the essence of this study. These are the behaviour of the patients and the behaviour of the staff. The behaviour of the patients is described as passive. A few exceptional cases are described which show personal growth following admission to a nursing home. It is shown that patients often ask for help to perform tasks which they are fully capable of doing themselves, because this is the only way they can obtain attention and social contact with the staff.

The behaviour of the staff, and the importance this has in maintaining a balance is described in accordance with the following subcategories. The staff's conceptions of dependency and independency, professional evaluation of the patient, goals and values, the staff's behaviour towards each other, the staff's own needs, task oriented or individually oriented nursing, foundation of knowledge, attitudes and stimulation measures.

Conclusion: To be able to maintain this, the staff must modify their own behaviour. They must also acquire more theoretical knowledge so that they can differentiate between different types of dependency. They will then be able to evaluate and thus meet the patient's real needs for help, on the basis of the patient's own needs and resources.
