Abstract
Hensikten med denne studien er å få økt kunnskap om hva sykepleiere i hjemmesykepleien dokumenterer på dagen, kvelden og natten. Videre er hensikten å drøfte funn i forhold til teori og tidligere forskning.
Det teoretiske rammeverket bygger på forskningsresultater og elementer fra sentrale teorier av relevans for problemstillingen.Det baseres på kunnskap knyttet til følgende områder: dokumentasjon, hjemmesykepleien, turnusarbeid, sykepleieprosessen, grunnleggende behov, pasientressurser og pasientproblemer.
Et deskriptiv design og kvalitativ og kvantitativ tilnærming er benyttet i studiens empiriske del. 25 pasienter, i 5 kommuner, samtykket i å stille sin dokumentasjon til rådighet for undersøkelsen. 7 pleieplaner, 57 dagrapporter, 9 aftenrapporter og 6 nattrapporter skrevet i en utvalgt måned ble analysert. Kategorisering ble foretatt på bakgrunn av funn, tidligere forskningsresultater og teori. Fysiske, psykiske og sosiale forhold ble vektlagt. Utdrag fra pleieplanene og pleierapportene presenteres for å understøtte funnene. Antall pleieplaner og pleierapporter som nevnte den enkelte kategori, ble talt opp og presentert i matriser.
Funn fra undersøkelsen viser at få pleieplaner var skrevet, og kun på dagen. Det var skrevet flest dagrapporter, få aftenrapporter og enda færre nattrapporter. Grunnleggende behov synes dokumentert gjennom hele døgnet. Fysiske forhold var nevnt i størst utstrekning, både i pleieplanene, og i dag-, aften- og nattrapportene. Psykiske forhold var også nevnt gjennom hele døgnet, men i liten utstrekning. Sosiale forhold var nevnt svært lite, og bare i enkelte pleieplaner og i dag- og nattrapporter. Ulike deler av sykepleieprosessen synes anvendt gjennom hele døgnet når sykepleiere dokumenterte.
The purpose of this study is to increase the knowledge about what nurses, working in home healthcare, document during the day, evening, and night. The purpose is also to discuss the findings connected to theory and earlier studies.
The theoretical framework has been built from scientific findings and elements from theories of relevance to this study. It is also based on knowledge about: Nursing documentation, home healthcare, shift-work, the nursing process, human needs, patient resources, and patientproblems.
A descriptive design with qualitative and quantitative elements were used in the empirical part of this study. 25 patients, from 5 districts, signed a written informed consent that their documentation was to be includet in this study. 7 careplans, 57 day-reports, 9 evening-reports and 6 night-reports, written during one month, were analysed. Categories were made, grounded on findings and earlier scientific results and elements from theory. Physical, psychological and social needs were emphasized. Scetches from the nursing careplans and the nursing reports are presented in the rapport to support the findings. Careplans and reports were counted and the number presented in matrixes.
Findings from this study show that just a few nursing careplans were written. These were written at daytime only. Most of the nursing reports were written at daytime, few in the evening and at night. Some of the patients basic needs seemes documented in the day, evening and night. Most of the nursing careplans and the reports written during the day, evening and night mentioned the physical component. The psycological component was also mentioned in documentation written during the day, evening and night, but less frequent. The social component was rarely mentioned, and only in the careplans, day-reports and night-reports. It seems like the nurses had used different parts of the nursing process when documenting during the day, evening and night.
