Abstract
Problemstilling:
Hva karakteriserer pasienters erfaringer med sykepleieres pasientundervisning?
Formål:
Målet med denne studien er å beskrive og få innsikt i hvilke erfaringer pasienter har med sykepleieres pasientundervisning. Slik kunnskap er viktig å identifisere for å utvikle kvaliteten av pasient- undervisning for å møte fremtidige pasienters undervisningsbehov. Den kan også gi ideer om hvilke forandringer som bør vektlegges i sykepleiepraksis og sykepleierutdanning for å imøtekomme pasienters behov for undervisning.
Teoretisk forankring:
I studien vektlegges Benner og Wrubels (1989) sykepleietilnærming, aktuell forskning og litteratur om pasientundervisning relatert til en didaktisk modell.
Metode:
Studien har et deskriptivt design. Det kvalitative forskningsintervju er valgt som data innsamlings- metode. Semistrukturerte intervjuer ble vurdert som relevante for at respondentene kunne beskrive sine erfaringer med pasient-undervisning. Individuelle intervjuer ble gjennomført med ni (9) voksne pasienter innlagt i en medisinsk avdeling i et somatisk sykehus. Dataanalysen ble utført etter Kvales (1981, 1984) tolkningsprinsipper i samsvar med tema og kategorier som ble synlige fra intervjuutskriftene.
Resultater:
Funn indikerer behov for pasientundervisning. Respondentene oppfatter pasientundervisning som informasjon, for å kunne ta ansvar for egen helse. De erfarer å ha et åpent tilbud fra sykepleierne om å spørre hvis det er noe de er i tvil om, derav studiens undertittel. Slik overlates initiativet om informasjon stort sett til pasienten selv. Respondentene ønsker dialog med sykepleierne. Respondentene gir uttrykk for at det er vanskelig å se forskjell på yrkesgruppene og å vite hvem de kan spørre om hva. De erfarer også at sykepleierne hovedsakelig overlater pasientundervisningen til legen. "Systemet er lagt opp til det", i følge en respondent. Respondentene beskriver pasientundervisningen fra sykepleierne som uforutsigbar, mangelfull og til dels personavhengig. I følge respondentene preges miljøet av sykepleiernes travelhet, lav prioritering av pasient- undervisning og lav sykepleiekontinuitet. Det gis klart uttrykk for at sykepleiere må vise bedre faglig og mellommenneskelig dyktighet i pasientundervisning for å fremme tillit, trygghet og likeverd.
Konklusjon:
Basert på funnene har sykepleierne behov for å utvikle en klar definisjon av sin rolle i pasientundervisning. Relatert til det må sykepleierens undervisende funksjon forbedres for å hjelpe pasienter å mestre det-å-være-syk og legge til rette for at pasienten kan ta ansvar for egen helse. Innholdet må være forutsigbart, meningsfullt, nødvendig og tilstrekkelig for pasienten. Varierte metoder anbefales. Samarbeid med andre profesjoner om pasientundervisning kan være hensiktsmessig. Sykepleieledelsen har ansvar for å sette pasientundervisning på dagsorden; organisere og legge til rette ressurser (personell, materiell). Sykepleierutdanningen har ansvar for å vektlegge teoretiske- og kliniske studier som kan hjelpe studentene å inkludere pasientundervisning i sykepleiepraksis. Det anbefales forskning om hvordan sykepleien kan organiseres for å ivareta pasienters behov for undervisning.
Problem statement:
What characterises patients' experiences of nurses' patient education?
Purpose:
The aim of this study is to describe and learn from which experiences patients have with nurses' patient education. Such knowledge is important to identify if the nurse is to develop the quality of patient education to meet futural patients' needs for education. It may also give ideas of what changes to stress in nursing practice and education to meet patients needs for education.
Theorethical framework:
This study focuses Benner and Wrubel's (1989) nursing theory approach, current research and literature about patient education related in a didactical model.
Method:
The study has a descriptive design. Qualitative research interviews were chosen as data collection method. Semistructured interviews were considered appropriate to allow respondents to describe their experiences of patient education. Individual interviews were conducted with nine (9) adult respondents hospitalized in a medical ward. Analysis of data was based on the interpretive priciples of Kvale (1981, 1984), involving the themes and categories that emerged from trascripts of the interviews.
Results:
Findings indicate needs for patient education. The respodents perceive patient education as information to be responsible for their own health. They have got an open offer from the nurse to ask if there is anything they want to know. Hence follows the subtitle of the study. Thus the initiative to patient education is left mainly to the patient himself. The respondents prefer dialogue with the nurse. They experience difficulties to distinguish the professionals, and especially knowing what professional to ask about which topic. They also find that nurses leave the patient education to the physician, and one respondent stated: "So is the system". The respondents describe the patient education as unpredictable, insufficient and partly dependent on the nurse. The ward athmosphere is characterised as busy for the nurses, low priority on patient education, and low degree of nursing continuity. It is evident, from the respondents point of view that higly skilled and human qualified nurses are needed in the patient education to encourage trust, safety, and equality.
Conclusions:
Based on the findings, nurses need to develop a clear definition of their role in patient education. Related to this, nurses should refine the teaching-coaching function of the nurse in helping patients to cope with illness, and to be responsible for his own health. The content should be predictable, relevant, and sufficient to the patient. Various methods are recomendable. Working closely with other professionals should be aproppriately. Nursing leadership should put patient education on the agenda, organise, and make resources accessible to patient education. Nursing educators need to ensure that they provide students with the course content and clinical experience that help them to incorporate patient education into nursing practice. Research must investigate how nursing should be organised to take care of patients needs for education.
