Abstract
Problemstilling: Gjør et holistisk eller et reduksjonistisk perspektiv noen forskjell for selve utøvelsen av sykepleie?
Formål: 1) Klargjøre begrepene holisme og reduksjonisme og identifisere om de har ulike konsekvenser for 2) kliniske vurderinger 3) aktiviteter og praktiske metoder
Teoretisk forankring: To forskjellige holistiske retninger er presentert av D.C. Phillips som vitenskapsteoretiske begreper. Disse holistiske retningenes betydning som ontologiske tilnærminger når det gjelder forholdet mellom helhet og deler er utledet. De to holistiske retninger danner sammen med filosofiske avklaringer av begrepene reduksjonisme, mekanisme og dualisme, fundament for analysen av sykepleielitteratur. Barbara Stevens Barnums' presentasjon av fire forskjellige tenkningsmønstre (dialektisk, logistisk, problematisk og operasjonell metode), er rammeverk for analysen av kliniske vurderinger. Analysen knyttet til aktiviteter og praktiske metoder, er fundert på to såkalt holistiske sykepleiemetoder, shiatzu akupress og terapeutisk berøring.
Metode: En teoretisk studie med en kritisk, dialektisk tilnærming.
Resultater: To forskjellige holistiske retninger er identifisert i sykepleielitteratur. Det har ikke vært mulig å identifisere forskjellige konsekvenser av de ulike holistiske retninger for kliniske vurderinger, eller for aktiviteter og praktiske metoder. I Callista Roys' og Martha Rogers' såkalte holistiske sykepleieteorier er det identifisert elementer av ideer som kan assosieres med såvel holisme som reduksjonisme. Teorienes beskrivelse av sykepleierens kliniske vurderinger og aktiviteter og praktiske metoder, kan ikke betraktes som avledet av noen av de holistiske retninger.
Konklusjon: Begrepene holisme og reduksjonisme er i sykepleielitteratur upresist definert, og krever ytterligere klargjøring. 1) De reelle forskjeller mellom de ulike holistiske retninger som ontologiske tilnærminger synes uklare. 2) Det er ikke mulig å identifisere forskjellige konsekvenser av de ulike holistiske retninger for kliniske vurderinger. Det er mulig å identifisere ideer innen den dialektiske og den logistiske metode som dels er forenlige med og dels er motstridende til de holistiske retninger. 3) Det er ikke mulig å identifisere noen forbindelseslinjer mellom de ulike holistiske retninger og aktiviteter og praktiske metoder. Denne studien av sykepleielitteratur gir ikke holdepunkter for at forskjellige konsekvenser av de ulike holistiske retninger kan utledes for sykepleierens utøvelse.
Research question: A holistic or a reductionistic perspective - does it make any difference for nursing practice?
Purpose: 1) Clarify the concepts of holism and reductionism and identify the eventual different consequences for 2) clinical judgement 3) activities and practical methods
Theoretical rationale: Two different holistic perspectives are presented by D.C. Phillips on a theory of science level. The meaning of these holistic perspectives as ontological approaches, with regard to wholes and parts is identified. The two holistic perspectives and reductionism, mechanism and dualism as philosophical concepts, are the foundation for the analysis of nursing litterature. Barbara Stevens Barnum's presentation of four different thought patterns (dialectic, logistic, problematic and operational methods) is the framework for the analysis of clinical judgement. Two so-called holistic nursing methods, shiatzu acupress and therapeutic touch, is the foundation for the analysis of activities and practical methods.
Method: A theoretical study with a critical and dialectical approach.
Results: Two different holistic perspectives are identified in nursing litterature. Different consequences of the two holistic perspectives could not be identified for clinical judgement, or for activities and practical methods. In Callista Roy's and Martha Rogers' so-called holistic theories, there are identified elements of ideas which can be assosiated with both holism and reductionism. The theories' descriptions of clinical judgement and activities and practical methods, cannot be derived from any of the holistic perspectives.
Conclusion: The concepts of holism and reductionism are unprecisely defined in nursing litterature and demand further clarification. 1) The genuine differences between the two holistic perspectives as ontological approaches are vague. 2) Different consequences of the two holistic perspectives for clinical judgement could not be identified. The two holistic perspectives are partly consistent and partly inconsistent with ideas from both the dialectic and the logistic method. 3) It is not possible to identify any connections between the two holistic perspectives and the activities and practical methods. This study of nursing litterature does not support the idea that different consequences of various holistic perspectives can be derived for nursing practice.
