Klassifikasjon av terapeutisk aktivitet i sykepleie : klassifikasjonssystemet NIC anvendt i en pilotstudie i psykiatrisk sykepleie

Abstract

Studiens hensikt:

Hensikten med studien er å få kunnskap om hvordan klassifikasjonssystemet NIC kan anvendes klinisk og om det gir en systematisk beskrivelse av sykepleiepraksis.

Metodevalg:

Studien har to deler. En kvalitativt beskrivende del med analyse av et intervju med en informant, innholdsanalyse av 164 tekstenheter produsert av samme informant med NIC som analyseinstrument, og tildeling av egenskaper til identifiserte NIC intervensjoner utover de som er gitt i manualen. En kvantitativt beskrivende del med måling av intervensjonenes gitte og tildelte egenskaper og en beskrivelse av kvantifiserte funn.

Studiens fokus:

Fokus er hvilke intervensjoner informanten utfører ift. en gitt pasientgruppe i type og antall. Videre i hvilken grad NIC kan si noe om innholdet i utført sykepleie, hvordan intervensjoner velges, individualiseres og utføres.

Funn:

Funn fra klassifiseringen med NIC som analyseinstrument gir en god beskrivelse av innholdet i sykepleien. 75% av sykepleien er rettet mot å bedre pasientenes psykososiale kapasitet. 17% er rettet mot kontroll av symptomer. 8% er indirekte terapeutisk aktivitet. 90% av sykepleien initieres av sykepleieren selv, mens 10% er ordinert. Klassifikasjonssystemet NIC viser at sykepleien har forskjellig design ift. hver enkelt pasient. Klassifikasjonen kan ikke si noe om selve utførelsen av intervensjoner.

The purpose of the study:

The purpose of the study is to gain knowledge of how to use the NIC classification system clinically and how NIC systematically can describe nursing practice.

Choice of metod:

The study has to parts. The first part consists of a qualitative descriptive part which contains a analysed interview with one nurse, content analysis of 164 units of text from the same nurse using NIC as analysing instrument and finally giving the interventions qualities which are not given in the manual. The second part consists of a quantitative part measuring the interventions qualities and a description of quantified findings.

Focus of the study:

The study focus is to register the interventions performed by the nurse related to a special group of patients. Further more to describe the content of nursing, how interventions are choosen and how these interventions are indiviualized.

Findings:

Findings from the NIC classification system described the content of nursing in a sufficient way. Results demonstrated that 75% of the nursing interventions were directed towards enhancing pshycosocial functioning, wheras 17% were directed towards controlling symptoms and 8% were focusesd on indirect therapeutic activity. The nurse initiates 90% of the nursing herself, whereas 10% of the interventions were initiated by the physician. The NIC classification system shows that the design of nursing treatment are different for each patient. The classification system can not be used to describe how the interventions are performed.
