Rapporten - arena for refleksjon?

Abstract

Hensikten:

Studiens hovedhensikt er å belyse den muntlige rapportens faktiske innhold i forhold til refleksjonen som foregår. Rapporten foregår i et fellesskap og fellesskapets betydning for refleksjonen vil utdypes.

Teoretisk forankring:

Den teoretiske forankringen har et multiperspektiv. Oppgaven bygger på en post positivistisk forståelse, preget av en hermeneutisk tankegang.

Metode:

Studien har et kvalitativt design, med feltmetodisk tilnærming. Problemstillingen er forsøkt besvart ved feltobservasjon og passiv deltagelse i sykepleierens muntlige rapporter på en medisinsk enhet. Fire sykepleiere fra samme enhet er gruppe-intervjuet. I analysen av lydbåndopptak og intervju anvendes analyseredskap innenfor Grounded Theory`s litteratur og hermeneutisk tolkningstradisjon.

Resultater og konklusjon:

Refleksjonen i den muntlige rapporten har i følge materialet fire hovedtendenser. Respondentene ble initiert til refleksjon ved opplevelse av uavklarte situasjoner, motsetningsforhold, utilfredstilte behov hos pasienten eller rammefaktorer. En annen tendens er at refleksjonen kommer til syne i den foregående dialogen, som viser seg i språket. Språket viser det i språkutveksling verbalt og nonverbalt, eller språkdrakten der monologen og samtalen tydeliggjør refleksjonen. Både monologen og samtalen skaper en refleksiv dialog ved sin formidlingsform, som er beskrivende eller fortellende. Dialogens reaksjonsformer, påvirker refleksjonen og gir uttrykk for forskjellig grad av refleksjon. Tredje tendens er resonnementet i refleksjonen, som sentralt er preget av sykepleierens analytiske resonnement. Det fremkommer som årsaksforklaringer, et forstående resonnement eller at sykepleieren vil "høre hva pasienten sier". Resonnementet gjøres mer eller mindre direkte, og er dominert av fortrolig erfaringsbasert kunnskap. Betegnende er også nytteverdien, som har mest form av normgivende handlingsforslag, men òg av at sykepleierne får økt innsikt og gir uttrykk for et moralsk ansvar. Konklusjonen i undersøkelsen er at refleksjonen i den muntlige rapporten er en sosial prosess og ikke bare individuell.

På grunnlag av disse funnene er det trukket ut mulige konsekvenser for sykepleien og antatte temaer for videre undersøkelser. Det kan ha betydning for både tilretteleggingen av, og refleksjonen i den muntlige rapporten.

Purpose of the study:

The principle purpose of this study is to describe the actual content of an oral report in connection with a nurse's reflections. The oral report takes place in a group situation (community) and the significance of a feeling of community will be elaborated.

Theoretical foundation:

The theoretical foundation reflects a multiple of theoretical perspectives. The study is based on a post positivistic understanding, as reflected in a hermeneutical way of thinking.

Method:

The study has a qualitative design including a field investigation approach. The research question has been attempted answered through field observations and passive participation in the nurses' oral reports on a medical ward. Four nurses from the same ward were interviewed in a group situation. Sound recordings of the interviews have been analyzed through Grounded Theory` tradition and the tradition of hermeneutical interpretation.

Results and conclusion:

Reflections in the oral reports contain four major tendencies in the material. Respondents are led to reflect by experiencing unresolved situations, opposing interests, the patient's unsatisfied needs or factors to do with the setting. Another tendency is that reflection takes place in the preceding dialogue which is apparent in the language that is used. The language exchanges may be either verbal or non-verbal. The style of the language may make reflection plain in the form of a monologue and/or a conversation. A reflective dialogue can be explicit in both a monologue and a conversation as a way of descriptive or narrative dissemination. The interactive reactions in a dialogue influence reflection and express different degrees of reflection. The third tendency is the line of argument in the reflection which is primarily influenced by the nurse's analytical argumentation. There is evidence of causal explanations, of comprehending argumentation or that the nurse wants to "hear what the patient has to say". Argumentation is done more or less directly, and is dominated by knowledge based on intimate experience. The utilitarian value is also apparent in the form of normative suggestions for action, in the nurse's increased insight and as an expression of the nurse's moral responsibility. The conclusion of the study is that reflection in the oral report is a social process and not just an individual process.

On the basis of these findings possible consequences for nursing have been drawn and themes for further studies are proposed. The findings of this study may be significant both for the preparation and the reflections in the oral report.
