Abstract
Formål med studien er å undersøke dokumentasjonen av systematisk symptomkartlegging av palliative pasienter i den elektroniske pasient journalen (EPJ).
Forankring i tidligere forskning. Systematisk symptomkarlegging beskrives som hjørnesteinen i palliasjon. Selvkartleggingsverktøyet ESAS skal sikre pasientens preferanser på de vanligste symptomene til kreftpasienter. Studier av dokumentert helsehjelp viser betydningen av bruk av symptomkartlegging og instrumenter. Dokumentasjonen av planlagt og utført helsehjelp vises å være mangelfull. Lokalt brukes standard ESAS til systematisk symptomkartlegging for å utløse DRG Z51.50.
Design og Metode. Dette er en registerstudie basert på en retrospektiv gransking av 570 pasientoppholds dokumentasjon i EPJ. I studien er det benyttet en deskriptiv og utforskende design. Oppholdene er identifisert basert på registrert bruk av DRG Z51.50 og/eller dokumentert brukt ESAS. Journalgranskingsinstrument er utarbeidet basert på de konseptuelle områdene fra ESAS skjemaet. I tillegg er det gjennomført en kvalitativ inspeksjon på 50 journaler der dokumentasjon med ESAS ikke er funnet.
Resultater. Bruk av ESAS - Audit viste at ESAS var i journalen i 160 av 570 journaler. Dette er avviker fra de lokale retningslinjene for dokumentasjon av systematisk symptomhåndtering for å utløse palliativ takst. Den kvalitative inspeksjonen viste at det var vanskelig å vurdere hvorvidt en systematisk symptomkartlegging var gjort eller ikke. Når ESAS er brukt - Pasientene er multisymptomatiske og har symptomer med høy skår. Fysiske symptomer er bedre dokumentert enn psykiske. Symptomer av moderat til alvorlig grad er bedre dokumentert enn symptomer av mild grad. Type dokumentasjon - Dokumentasjonsformen er i overvekt problemløsning ved moderat til alvorlig grad av symptomer, spesielt i forhold til symptomer av fysisk karakter. Symptomer av psykisk karakter er i hovedvekt dokumentert som observert eller kartlagt. Standardiserte maler i journalen førte til at symptomene i noen grad var nevnt, men pasientpreferansene manglet som oftest.
Konklusjon. Det er behov for en bedret dokumentasjon av systematisk symptomkartlegging i EPJ. Funnene tyder på at samsvaret på pasientens rapporterte symptomer og helsepersonells dokumentasjon er noe bedre i denne studien der ESAS er brukt enn på samsvarstudier der helsepersonell ikke har hatt tilgang til det systematiske symptomkartleggingsverktøyet. Der ESAS er brukt er det imidlertid et forbedringspotensiale i å bruke pasientinformasjonen mer aktivt i dokumentasjonen av planlagt og utført helsehjelp.

The main purpose of this study is to examine the documentation of systematic mapping of symptoms of palliative patients in the electronic patient journal (EPJ).
Anchor in previous research: Systematic mapping of symptoms is described as the cornerstone of palliation. The selfmapping instrument ESAS shal guarantee patient preferences of the most usual symptoms. Studies of documented health care show the significance of mapping of symptoms and instrument. Documentation of planned and carried out healthcare is shown to be insufficient. The standard of ESAS is used locally for systematic mapping of symptoms to release DRG Z51.50.
Design and Method: This is a register research study based on a retrospective investigation of 570 hospitalised patients and the documentation of them in the EPJ. In the study there has been used a descriptive and explorative design. The hospitalisation is identified based on registered use of DRG Z51.50 and/or documented use of ESAS. The instrument of investigation of the journal is composed based on the conseptual areas from the ESAS questionnaire. There is additionally carried out a qualitative inspection of 50 journals where the documentation of ESAS is not found.
Results: The use of ESAS. The audit showed that ESAS was in 160 of 570 journals. This deviate from the local guidlines for documentation of systematic management of symptoms in order to release palliative estimated value. The qualitative inspection showed that it was difficult to judge if a systematic mapping of symptoms was done or not. When ESAS is used. The patients are multisymptomatic and have symptoms with high scores. Physical symptoms are better documented than psycological ones. Symptoms of a moderate to serious degree are better documented than symptoms of a low degree. Type of documentation. The form of documentation is mainly problemsolving when symptoms are from a moderate to a serious degree, especially if the symptoms are of a physical character. The symptoms of a psycological character are mainly documented as observed or mapped. Standardised working models in the journal led to lack of registered symptoms and the patient preferences were almost always lacking.
Conclucion: There is a need for bettered documentation of the systematic mapping in EPJ. Findings in this study where ESAS is used show that symptoms reported by the patients were more in accordance with the documentation of the health personnel than in corresponding studies where health personnel did not have access to the systematic symptom mapping instrument. Where ESAS is used, however there is a potential for improvment if the patient information is used more actively in the documentation of planned and carried out health care.
