Abstract
Formål: Formålet med denne studien var å kartlegge endring i albuefunksjon etter kirurgisk behandling av olecranonfraktur og hvilke endringer i leddbevegelighet, muskelstyrke, grepskraft og smerteintensitet som var assosiert med en endring av tilfredshet med albuefunksjon fra tre måneder til ett år. Bakgrunn: Rehabilitering etter albuebrudd kan ha et langvarig og utfordrende forløp. Eksisterende fysioterapitiltak er i stor grad basert på empiri og pasientens funksjonskrav. Utfordringene ved redusert albuefunksjon og hvilke funksjonskrav som kreves for normal albuefunksjon er velkjent. Derimot er endringen i albuefunksjon etter kirurgi ved olecranonfraktur lite undersøkt og hvilke funksjonsparametre som assosieres med pasientens tilfredshet er ikke dokumentert. Metode: Studien baseres på datamateriale fra en multisenter randomisert kontrollert studie, Clinical Randomized Olecranon Fracture Trial (CROFT). Utvalget bestod av 46 deltakere med kirurgisk behandlet olecranonfraktur. Det primære utfallsmålet var tilfredshet med albuefunksjon målt med modifisert VAS fra 0-100 (VAS-101). Leddbevegelighet, muskelkraft, grepskraft og smerte ble registrert tre måneder og ett år postoperativt. Univariate analyser og multippel lineær regresjonsanalyse ble gjennomført for å kartlegge endring i albuefunksjon og hvilke funksjonsendringer som var assosiert med en endring av tilfredshet med albuefunksjon. Resultater: Resultatene viste en gjennomsnittlig økning i tilfredshet med albuefunksjon på 8,9 på modifisert VAS-101 (95 % KI: 4,5, 13,3, d = 0,60, p = ,001) fra tre måneder til ett år. Økt fleksjonsbevegelighet (B = 0,43 [95 % KI 0,19, 0,68], p = ,001, β = 0,45) og reduksjon i smerteintensitet (B = -0,45 [95 % KI -0,15, -0,74], p = ,004, β = -0,34) bidro mest til å forklare økningen i tilfredshet med albuefunksjon. Den forklarte variansen i regresjonsmodellen var 62 %. Resultater fra parede T-tester viste en gjennomsnittlig økning i ekstensjonskraft på 18,8 Newton (95 % KI: 7,5, 30,1, d = 0,54, p = ,002), fleksjonskraft på 21,3 Newton (95 % KI: 5,6, 37,1, d = 0,44, p = ,009), ekstensjonsbevegelighet på 7,3 grader (95 % KI: -4,4, -10,3, d = 0,77, p = <,001), fleksjonsbevegelighet på 9,0 grader (95 % KI: 4,4, 13,6, d = 0,58, p = <,001) og en gjennomsnittlig reduksjon i smerteintensitet på -4,2 på modifisert VAS-101 (95 % KI: -7,6, -0,7,
d = 0,36, p = ,018) fra tre måneder til ett år. Ingen signifikante endringer ble observert for grepskraft, pronasjons- og supinasjonsbevegelighet i samme tidsrom. Konklusjon: Tilfredshet med albuefunksjon viste en signifikant økning fra tre måneder til ett år etter kirurgi. Reduksjon i smerteintensitet og økning i fleksjonsbevegelighet var de funksjonsvariablene som var assosiert med økt tilfredshet med albuefunksjon fra tre måneder til ett år etter kirurgi. Resultatene viste moderate endringer i fleksjons- og ekstensjonsbevegelighet, fleksjons- og ekstensjonskraft og smerteintensitet i samme periode.
Purpose: The aim of the present study was to assess changes in elbow function following surgical treatment of olecranon fractures, and to identify changes in joint range of motion, muscle strength, grip strength, and pain intensity associated with an increase in satisfaction with elbow function from three months to one year post-surgery. Background: Rehabilitation following an elbow fracture can be a prolonged and challenging process. Existing physiotherapy treatment programs are largely based on empirical evidence and the individual patient’s functional requirements. The challenges associated with reduced elbow function and the functional requirements for normal elbow function are well known. However, little research has been conducted concerning the changes in elbow function following surgery after an olecranon fracture, and the functional parameters associated with patient satisfaction are unknown. Method: The study was based on data from a multicenter randomized controlled trial, the Clinical Randomized Olecranon Fracture Trial (CROFT). The sample consisted of 46 participants with surgically treated olecranon fractures. The primary outcome measure was satisfaction with elbow function, assessed using a modified VAS scale ranging from 0-100 (VAS-101). Joint mobility, muscle strength, grip strength, and pain were recorded at three months and one year post-surgery. Univariate analyses and multiple linear regression analyses were conducted to identify changes in elbow function and identifying functional parameters associated with changes in satisfaction with elbow function. Results: The results showed a mean increase in satisfaction with elbow function of 8.9 on the modified VAS-101 (95% CI: 4.5, 13.3, d = 0.60, p = 0.001) from three months to one year. Increased flexion range of motion (B = 0.43 [95% CI: 0.19, 0.68], p = 0.001, β = 0.45) and reduction in pain intensity (B = -0.45 [95% CI: -0.15, -0.74], p = 0.004, β = -0.34) contributed the most to explain an increase in satisfaction with elbow function. The explained variance in the regression model was 62%. Paired t-tests showed a mean increase in extension force of 18.8 Newtons (95% CI: 7.5, 30.1, d = 0.54, p = 0.002), flexion force of 21.3 Newtons (95% CI: 5.6, 37.1, d = 0.44, p = 0.009), extension range of motion of 7.3 degrees (95% CI: -4.4, -10.3, d = 0.77, p < 0.001), flexion range of motion of 9.0 degrees (95% CI: 4.4, 13.6, d = 0.58, p < 0.001), and a mean reduction in pain intensity of -4.2 on the modified VAS-101 (95 % CI: -7.6, -0.7, d = 0.36,
p = 0.018) from three months to one year. No significant changes were observed for grip strength, pronation or supination during the same period. Conclusion: There was a significant increase in satisfaction with elbow function from three months to one year post-surgery. A reduction in pain intensity and an increase in flexion range of motion were the most strongly associated variables with increased satisfaction with elbow function from three months to one year. The results demonstrated moderate changes in flexion and extension range of motion, flexion and extension strength, and pain intensity during the same period
